Athletic Clearance 

Spring athlete clearance forms deadline: February 9, 2018

Each year, all Central Catholic student athletes must complete the following items before they are eligible to participate in any tryouts, practices, or games.

Central Catholic Codes of Conduct: CLICK HERE

  • Two forms: one for student-athlete, one for parent/gaurdian. Both must be completed once each school year.

OSAA Sports Physical Exam:  CLICK HERE

  • Physicals are good for two years and are normally completed prior to the Freshman and Junior years
  • If a physical expires, a new one must be completed before the start of the season
  • Please turn in completed form to Rick Potter in the athletic office

Pre-Participation Private School Informed Consent Concussion form: CLICK HERE 

  • Must be completed every year
  • Please turn in completed form to Rick Potter in the athletic office

OSAA Eligible Student Transfer Form: CLICK HERE

  • Must be completed by all students transferring to Central Catholic from another high school
