Athletic Clearance Forms Must be Completed Once Every School Year
TRYOUTS FOR WINTER SPORTS START November 17, 2025
Any student-athlete interested in participating in a fall sport must complete all of the clearance requirements no later than Friday, November 14 at 5:00 P.M. to be eligible to participate in any practices, workouts, or tryouts.
Please use this link to complete the Athletic Clearance process, noting:
- You will be asked to upload your OSAA Sports Physical Exam Form. If you do not upload it as a part of the clearance form submission process, you will need to email a PDF of the completed form to Chris Palmer '14 at cpalmer@centralcatholichigh.org prior to the start of the season.
- Please name the file following this standard: last name first name OSAA Physical Exam (e.g. SmithWillOSAAPhysicalExam).
- Your physical exam form is good for two years from the date of the exam.

All transfer student athletes are required to submit an OSAA Eligible Student Transfer form. If you do not upload the transfer form as a part of the clearance form submission process, you will need to email a PDF of it to Chris Palmer '14 at cpalmer@centralcatholichigh.org prior to the start of the season.
