Athletic Clearance Forms Must be Completed Once Every School Year

TRYOUTS FOR WINTER SPORTS START November 17, 2025

Any student-athlete interested in participating in a fall sport must complete all of the clearance requirements no later than Friday, November 14 at 5:00 P.M. to be eligible to participate in any practices, workouts, or tryouts.

Please use this link to complete the Athletic Clearance process, noting:

• You will be asked to upload your OSAA Sports Physical Exam Form. If you do not upload it as a part of the clearance form submission process, you will need to email a PDF of the completed form to Chris Palmer '14 at cpalmer@centralcatholichigh.org prior to the start of the season.
  • Please name the file following this standard: last name first name OSAA Physical Exam (e.g. SmithWillOSAAPhysicalExam).
  • Your physical exam form is good for two years from the date of the exam.

• All transfer student athletes are required to submit an OSAA Eligible Student Transfer form. If you do not upload the transfer form as a part of the clearance form submission process, you will need to email a PDF of it to Chris Palmer '14 at cpalmer@centralcatholichigh.org prior to the start of the season.

  • Please name the file following this standard: last name first name OSAA Transfer Form (e.g. SmithJaneOSAATransferForm).
