Athletic Registration and Clearance Forms Must be Completed Once Every School Year, While Oregon State Laws Require the Physical Exam Form to be Completed Every Two Years

Central Catholic Athletics is proud to partner with FinalForms, an online registration and forms management system that simplifies the athletic clearance process for families.
Whether your student plans to participate in athletics this year—or is still deciding—we encourage you to create a FinalForms account and complete the registration process as soon as possible.
Please note: All student-athletes must be cleared through FinalForms before participating in any official practices, workouts, or tryouts. Completing your registration also notifies our coaching staff of your student's interest in participating. It serves as the primary communication platform used by coaches and the Athletics Department throughout the school year.
Any student-athlete interested in participating in a fall sport must complete all athletic clearance requirements no later than Thursday, August 13 at 4:00 p.m. to be eligible for fall practices, workouts, and tryouts.
