Basic Information/Placement Exam Registration Form
Complete and mail in the Basic Information/Placement Exam Registration Form with a $20 fee. You may also register for the exam online at www.centralcatholichigh.org after October31. Registration confirmation will be sent once payment is received.

Parents are to hand the signed Records Release form to the principal or secretary of the applicant's current school and ask that all the student's test scores, report cards, and transcript information (copies only, not the originals) be mailed directly to the Central Catholic Admissions Office address noted on the form. Please note: Since Central Catholic would like to receive the student's grades for the first semester of the 8th grade year, school secretaries or registrars should not send student's records until those grades are in. Parents are asked to respect this timetable, since there will be no benefit to submitting a student's academic records before the January 30, 2009 deadline.
