Athletic Clearance 

2017-2018 Clearance Forms

Each year, all Central Catholic student athletes must complete the following items before they are eligible to participate in any sports tryout, practice, game, etc.

Return all forms to Rick Potter in the Athletic Department. (Code of Conduct forms are completed and submitted on-line)

Clearance Deadlines
Fall Sports August 7, 2017

Central Catholic Codes of Conduct: 
The student athlete and parent/guardian must complete these forms annually:  CLICK HERE

OSAA Sports Physical Exam:
If your physical expires during the season a new one must be completed before the start of the season (physicals are good for two years and are normally completed prior to Freshman and Junior years).  
All student-athletes must have a current OSAA School Sports Physical exam on file. (Valid for two years from the date they are issued).  CLICK HERE

Concussion Private School Informed Consent form: 
The student athlete and parent must complete this form annually:  CLICK HERE 

OSAA Eligible Student Transfer Form
All students transferring to Central Catholic from another high school must complete this form CLICK HERE.
