2010-11 Sports Clearance Task List
All CCHS Student Athletes must complete the following five items (if applicable) before they are eligible to participate in tryouts, practices, games, etc. For Fall 2010 Sports these items must be completed before August 20, 2010 in order to be eligible to start tryouts on August 23, 2010.
The Code of Conduct and Athletic Emergency forms are completed on-line and submitted electronically to the athletic department. The other forms are downloaded, completed and delivered or mailed to the athletic department or submitted via fax (Fax number: 503-961-9906)
1) All incoming Freshman, 2009-10 transfer students and all Sophomore, Junior or Senior student-athletes who did not have a sports physical during the 2009-10 school year must complete a Sports Physical and deliver it or fax it (503-961-9906) to the Athletic Department. 2) An online CCHS Athletic Code of Conduct. must be read and acceptance form must be submitted to the Athletic Department. Click here to read and complete the online form.
3) An online emergency information form must be competed and submitted to the Athletic Department. Click here for the CCHS Athletic Emergency Form.
4) All transfer students (transferring from another high school...not entering as Freshmen) must complete an OSAA Eligible Student Transfer Certificate and bring it to the Athletic Department. Click here for a copy of the OSAA Transfer Form.
5) During the school year, transportation to practices, games, etc. is provided by CCHS. A student riding with their own parent/guardian to or from an event must notify their coach. To ride with any other person including other students, other parents, etc. a transportation waiver must be on file with the athletic office (deliver it or fax it (503-961-9906) to the Athletic Department).
